Stock Certificate

A missing or destroyed stock certificate should be reported immediately to Mellon Investor Services . The notification should include the owner's name, social security or taxpayer identification number, address, specific information about the certificate (certificate number, number of shares, and issue date, if available), whether or not the certificate was endorsed, and a daytime telephone number.

To place an immediate stop on the transfer of a lost or stolen stock certificate, please call Mellon's toll-free telephone number, 1-800-719-9056, and one of its customer service representatives will assist you. If the certificate is later found or recovered, the securityholder should notify Mellon Investor Services in writing so that the "stop transfer" may be removed.
